Minnesota’s Immunization Law and Exemption Provision
Scroll to the bottom and click the links for “Forms”. You will then be redirected to the page below. You then click the link as seen circled below.
Once you download the form this is what it will look like. It is pretty quick and straight forward from here. You simply fill it out, get it notarized, and hand it over to whomever is requiring it whether school or daycare provider. Before you do that, make sure to make a copy for your records just in case!
